CANCELLATION POLICY-please cancel 24 hours prior by phone or email, or there will be $35 fee

Work forms/ Dr forms $25/page (may increase, at discretion of treating physiotherapist)

Therabands $2/meter (for patients only

Miniband $4 (for patients only)

Copy of notes or page $2/page

If your spouse has a 2nd insurer that is different from yours for example, you have Bluecross and your spouse has Great West Life, sorry we can only bill to one insurance company. When the first insurer has reached the maximum then we can bill to the 2nd insurer.
